An application linked to care management system. This mobile application has 3 different versions for patients, providers and care givers. The main goal of this application is to connect among them with the right information through the system including Telemedicine, checking health information instantly.
Main > The main screen show upcoming, past due completed task that a patient needs to do. All the tasks are managed by care managers and care givers. By notification, the app provides appointment, the result of imaging, cost, doctor’s note, recommended activity, food and others related to health care.
Main Desktop Version > The application provides desktop, mobile and App.
health360 > This screen show all the information related to a patient such as Encounters, Conditions, Medications, Observation, Assessments, Imaging, Immunization, Procedures, and allergies.
Other screens > In this app, there are Care Plan, Chatting, Timeline, RPM for patients, caregivers and providers.
