The system for Care Manager allows to check patient’s health information with all care members related to the patient. The system has Care Plan, Case Request, health360, RPM, Notes, Telemedicine and more screens to manage. The solution is called “careassistant.ai”
Dashboard > This is a first screen when a care manager log in. The screen shows kpi cards which is located on the top including the number of new/active patients, active cases, tasks, assessments and events. And other sections shows several information related to a patient with graph, timeline cards and calendar.

health360 > The screen shows the details of patient such as conditions, encounters, risk scores, medications, observations, imaging, assessments, allergies, immunization and procedures. And each card has “Details” CTA, once an user clicks it, the screen goes to the detail screen.


Care Plan > This is a screen following a patient closely with goals, barrier, action, intervention and note. Care Manager can add a new problem, and the new problem is able to be generated by AI.

Telemedicine > This application allow to patient and care members to chat and talk through a camera.


Other screens > Assessments, Case Request, Chat, Note, Patient List, Payer Analytics, Pop360, RPM. Timeline and Utilization Management
